Registered Nurse – Home Health

Location: Winston-Salem, NC

Shift: 8-hour shifts

Hours: 40 hours/week

Job Summary:

The Home Health Registered Nurse provides skilled nursing care to patients of all ages in their homes. This role is responsible for patient assessment, development and evaluation of individualized care plans, coordination with the interdisciplinary team, patient/family education, and ensuring high-quality, continuous care in the home setting.

Key Responsibilities

• Perform comprehensive assessments in both stable and emergent home health situations.
• Develop, implement, and update individualized Plans of Care (POC).
• Document all patient visits, changes, and care decisions accurately and promptly.
• Provide skilled nursing interventions including wound care, IV therapy, phlebotomy, catheter care, and disease management.
• Educate patients and families on medications, treatments, safety, and self-care strategies.
• Coordinate services with the interdisciplinary team, physicians, and community agencies.
• Supervise HHAs/LVNs as needed to ensure quality care delivery.
• Participate in on-call rotation and interdisciplinary team meetings.
• Ensure compliance with payer requirements and infection control standards.
• Required Skills & Competencies
• Strong clinical assessment, critical thinking, and communication skills.
• Proficiency with Homecare Homebase and OASIS documentation.
• Competence in wound care, IV therapy, phlebotomy, respiratory care, and durable medical equipment.
• Ability to work independently, manage time effectively, and function under pressure.
• Reliability, professionalism, and the ability to maintain patient confidentiality.

Qualifications:

• Current state Registered Nurse (RN) license – Required
• 1–2 years clinical RN experience – Required
• 2+ years home health experience – Required
• Homecare Homebase experience – Required
• OASIS experience – Required
• CHPN certification – Preferred
• CPR certification – Required
• Valid driver’s license, reliable transportation, and insurance – Required
• ADN or Nursing Diploma – Required
• BSN – Preferred
